Presenting a case of extremely rare occurrence: extramammary Paget's Disease of perianal topography secondary to local resection of a villous polyp of the rectum undergoing adenocarcinomatous change; clinical and anatomopathological characteristics of this disease and treatment of same. The conclusion drawn ís that it ís always necessary to consider this type of lesion as a true cutaneous indicator of the starting point of a cancer in apocríne glands in the area or a hidden rectal cancer.
